Travel Advice and Vaccinations

Before travelling, patients are required to complete our Travel Risk Assessment Form. Once submitted, the form will be reviewed by our nursing team (Jayne or Maria).

After reviewing your information, the nurses will determine whether any travel vaccinations are required.

  • If vaccinations are needed, our reception team will contact you to arrange an appointment.

  • If no vaccinations are required, a member of the team will give you a brief call to provide travel health advice.

We ask that you submit your form at least 6 weeks before your travel date. This allows sufficient time for us to review your information, arrange appointments if necessary, and order any required vaccines.

However, if you submit your form with less than 6 weeks’ notice, we will still review it and do our best to offer an appointment if nurse availability allows.

Please complete our Travel Risk Assessment Form using the link above. This gives our nursing team the information they need to assess your travel health requirements.

What happens after I submit the form?
Once completed, the form will be reviewed by our nurses (Jayne or Maria). They will assess whether any travel vaccinations are required.

Will I be contacted after submitting the form?
Yes.

  • If vaccinations are required, our reception team will contact you to arrange an appointment.

  • If no vaccinations are needed, a member of the team will call you to provide travel health advice.

Why do I need to submit the form 6 weeks before travelling?
We ask for at least 6 weeks’ notice to allow enough time to review your form, arrange any appointments, and order vaccines if needed.

What if I am travelling in less than 6 weeks?
You should still complete the form. If the nurses have availability, we will do our best to arrange an appointment before you travel.

Are there any costs for travel vaccines?
Some travel vaccines are free on the NHS, while others must be paid for privately. If a vaccine is not available on the NHS, we will inform you of the cost before arranging your appointment.
